Georgia state law requires all seventh graders, born on or after January 1, 2002, have a Meningitis (MCV4) and a Tetanus, Diphtheria, and Pertussis (Tdap) vaccinedocumented on a Georgia 3231 Immunization form before entering the seventh grade.

Due to state law requirement, the Georgia Department of Public Health Certificate of Immunization (Form 3231) is proof of meeting immunization requirements. Your child must have documentation of having both vaccines completed before the first day of school. Seventh graders must have both vaccines to receive a schedule and attend class. Exceptions to this requirement are documented exemptions.
